Privacy Notice

This notice explains what information the Vortex Membership portal (portal.vortexmsinc.com) collects, why, and how it is protected. The portal is operated by Vortex Management Services Inc., 12950 SW 133rd Ct, Miami, FL 33186.

Your medical records are not kept in this portal. They are kept in our electronic health record system and are protected under HIPAA, as described in our Notice of Privacy Practices, which you can request at the front desk or at contact@vortexmsinc.com.

Information we collect

Account holder: name, date of birth, sex, email, phone, mailing address and a password (stored only as a one-way hash).
Family members you enroll: name, date of birth, sex and relationship to you. Sex is used only to choose the picture on the member card.
Your agreement: the exact text you signed, your typed signature, the date and time, your IP address and your device type, so the signature can be verified.
Payments: handled by Stripe, Inc. We never see or store your full card number. We keep the card brand, the last 4 digits, the expiration date and whether it is credit or debit (to apply the credit card fee).
Security records: sign-ins, member checks at the front desk and changes to your membership, with the date, time and IP address.

How we use it

To create and manage your membership, issue member cards and confirm your membership at each visit.
To charge the membership fees you authorized and send receipts and membership notices.
To keep the portal secure and to meet our legal and record-keeping duties.

We do not sell or rent your information, and we do not use it for advertising.

Who we share it with

Stripe, Inc., to process payments.
Cloudflare, Inc., which hosts the portal and sends our emails.
Government agencies or courts when the law requires it.

Cookies

The portal uses only the cookies needed to keep you signed in and to finish a payment. We do not use advertising or tracking cookies.

Children

You must be 18 or older to create an account. A child under 18 can only be enrolled as a family member by a parent or legal guardian, who manages the membership. We do not knowingly collect information online directly from children.

How we protect it

All connections are encrypted. Staff access requires a password and a two-factor code, each person only sees what their role needs, and every check and change is logged. Password reset links work once and expire in 30 minutes.

How long we keep it

We keep membership and agreement records while your membership is active and for at least 5 years after it ends, or longer when a law requires it. Payment records are kept as tax law requires.

Your choices

See your membership, cards and signed agreement at any time in the portal, and download the agreement as a PDF.
Ask us to correct your information, or for a paper copy of your agreement at no charge.
Withdraw your consent to receive documents electronically. We will then send them by mail.
Cancel your membership with 30 days notice, as described in your agreement.

If there is a security incident

If personal information is accessed without authorization, we will notify the people affected as required by Florida law (section 501.171, Florida Statutes) and, for health information, by HIPAA.
