Example: Carlos signs up for Bronze. His card shows plan BRONZE. This month he uses his 1 visit and his blood draw. A week later he has a question and books a Televisit for $25, since his monthly visit is already used.

Every family member gets their own card. The primary subscriber's card is marked Subscriber; each dependent's card is marked Dependent and shows who their subscriber is.
TEX plus a short number assigned in order. The subscriber always ends in 00, each dependent adds 01, 02, 03.

The mascot is chosen by the member's age and sex on file, not by their role. A spouse or an adult child who is a dependent still gets an adult mascot; the boy and girl mascots are only used for minors, under 18 per U.S. law.
